On 7/7/20 at 1:07 PM, Licensing Program Analyst (LPA) Keturah Lane, conducted an announced Case Management Tele-Inspection visit regarding a day care parent who tested positive for COVID-19. Due to COVID-19, a tele-inspection was conducted using Zoom to tour the facility. Director Crystal Burroughs and Erin Flores were present at the tele-inspection. Census at time of report was 28 children. Director stated that she contacted the Dept. of Health and left two messages.

Director stated that the parent was last at the facility on 7/2/20 and tested for COVID-19 on 7/2/20. The family informed the Director of the positive test result on 7/3/20. The facility was closed on 7/3/20. The child was last in care on 7/2/20 and was dropped off the mother and picked up by the father. The children and staff member were with a stable group of five children on 7/2/20 in the Green Room. The Green classroom has three groups of children in the same room that are separated by shelving. There was a total of 18 children in the classroom on 7/2/20. Census for the entire facility on 7/2/20 was 35 children. The facility also has an infant/toddler license as well. Census for the infant/toddler license on 7/2/20 was 6 children.

Director had the facility deeply sanitized on 7/3/20 and over the holiday weekend. Director notified all the families and advised staff members to monitor themselves during the next 14 days for symptoms. Director changed the drop off and pick up procedures by having staff members pick up children from cars in the parking lot. Director reported that the new protocol went well. LPA Lane will continue to follow up with Director Burroughs on the facility's situation. LPA received the written LIC624 report and Child roster as requested on 7/6/20.

An exit interview was conducted with the Director. Appeal Rights were discussed and provided. Facility was advised to post the Notice of Site Visit for 30 days. A copy of the report will be e-mailed to the facility and director was advised that acknowledgement of the receipt of the report is to be received within twenty-four hours.
