On 8/4/23 at 1:45pm, Licensing Program Analyst (LPA), Martha Malane arrived at the facility to conduct a case management inspection for the purpose of a capacity decrease and room change. Upon arrival, LPA met with Director, Delisha Doakes and Assistant Director, Thanh (Pham) Warrens and was led on a of tour the facility. There were no children as the facility is closed for training week. The facility is requesting a capacity decrease to 12 infant children (age 12-24 months) in room 112 and to remove rooms 123 and 124 from the license. Hours of operation are Monday through Friday 7:00am to 5:30pm.

The indoor space measured a total of 1070.71 square feet which is sufficient to accommodate 30 children, however licensee is only requesting a capacity of 12 children. Room 112 is equipped with appropriate furniture, including mats for napping, tables, chairs, cubbies, bookshelves and other materials for children. The facility has a total of three (3) toilets and two (2) sinks for the children's exclusive use, which is sufficient to accommodate the requested capacity. There is a sink within arm’s reach of the changing tables. Director stated she understands the sink near the diaper changing table shall not be used for food preparation.

The outdoor activity space was inspected and measured a total of 949.11 square feet which is sufficient to accommodate the requested capacity of 12 infant children.

A fire clearance for 24 infant children (age 12-24 months) in room 112 was granted 7/28/23.

A capacity decrease for 12 infant children (age 12-24 months) in room 112 and the removal of rooms 123 and 124 is granted as of today’s date, 8/4/23.



Exit interview conducted with Director, Delisha Doakes and Assistant Director, Thanh (Pham) Warrens. Notice Site Visit shall be posted for 30 days from today's date. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
