An Informal Conference was held in the Riverside Child Care Regional Office (RCCRO) with the Boys and Girls Clubs of Greater San Diego and Riverside Child Care Regional Office staff. Licensing Program Manager I (LPMI) Kimberly Williams and Licensing Program Analyst (LPA) Laura Callahan conducted the meeting with the facility director, Olga Camacho. The purpose of this meeting is to address concerns regarding previous violations of Title 22 and Health and Safety Regulations, including the Responsibility of Providing Care and Supervision.

The following history of citations which occurred within the last three years were discussed:

101229(a)(1) Care and Supervision (UIR)- On 11/15/18, at approximately 11:45AM, staff # 1 left child # 1 on the playground without supervision and Staff # 2 did not immediately attend to the child when no teacher was present, and waited two to three minutes before reporting the incident.

101229(a)(1) Care and Supervision (Complaint) - On or about 06/20/18 at approximately lunch time, Staff was unaware that Child #1 was alone on the. This violation poises an immediate health and safety risk to the children in care. 101212(d)(1)(c) Reporting - The facility failed to report the incident involving lack of supervision to the Licensing Agency within 24 hours.

101216(a) Personnel Requirements (Complaint) - On a visit conducted on 02/23/17, the facility received a citation for not being able to meet the individual needs of children in care - there was a lapse in the continuity of care for child#1 and staff did not adequately meet the child's needs.

101229(a)(1) Care and Supervision (Complaint) - On a visit conducted on 11/17/16, the facility received a citation for a child receiving unexplained injuries (injuries consistent with a fall).

LPM I Williams reminded the Facility representative, Director Olga Camacho, of how important regulatory compliance is in licensed facilities to protect the Health and Safety of children in care. The Director has been informed the Licensing Agency takes these violations seriously, and future violations may lead to a Non Compliance Conference and/or the Department seeking legal consultation regarding the status of the license

The Director stated she has implemented new procedures to prevent future incidents and provided copies of documentation to support these new procedures. The Director provided copies of agendas from the Active Supervision training conducted on 10/03/19 & 01/21/19 for all staff. She has also implemented a Daily Head Count/ Name to Face process which keeps accurate counts throughout the day implemented 10/03/19. The Director stated that lead teachers also carry walkie-talkies to communicate with other staff. The Director stated that she understood what was discussed during the conference and has agreed to operate the facility in substantial compliance with all Title 22 regulations and Health and Safety codes.

As a result of the informal conference, the Facility will complete and understand the following:

1. Facility agrees to maintain a daily log/journal of all classroom's observations conducted at least on a monthly basis. The log shall be made available for the Department's review upon request.
2. Facility will continue to provide Personnel Reports (LIC 500) on a quarterly basis.
3. Facility understands that working cooperatively and with transparency with the Department in expected.
4. Facility understands the Department may be seeking legal consultation regarding the facility’s prior and recent issues.
