On March 30, 2022, at 8:52 a.m., Licensing Program Analysts (LPAs), Cindy Hamilton and Anastasia Flores met with North Coast Church Preschool (CCC) Director Marcia Boeche to deliver the findings for the above stated allegations.  During the investigation, LPA Hamilton conducted interviews with three children, six parents and four staff.  LPA Hamilton and LPA Nasha King conducted a health and safety inspection of the CCC on February 16, 2022 and no safety concerns were noted.  LPAs obtained and reviewed the facility’s class rosters, Unusual Incident Reports (UIRs), and other pertinent documentation.

On February 10, 2022, Community Care Licensing (CCL) received information stating that staff is not practicing PPE, licensee did not notify parents of positive COVID cases and Staff #1 (S1) handled day care child in a rough manner.  Regarding the allegation staff is not practicing PPE, it was alleged that children and staff do not consistently wear masks indoors. Confidential interviews resulted in LPA being provided conflicting information. facility staff, parents, and children always wear masks while indoors. Confidential interviews and record reviews also revealed that the facility did inform parents and staff of the mask mandate which stated that everyone over the age of two years old must always wear a mask while indoors and the mask mandate was being enforced by CCC. On February 16, 2022, LPAs conducted a health and safety inspection of the CCC and LPAs observed all adults and children inside the facility and those entering the facility to be properly masked or wearing a face covering. In addition, LPAs observed signage on the front doors of the facility advising those who entered that a mask is always required to be worn while inside the facility.

Regarding the allegation, licensee did not notify parents of positive COVID cases, it was alleged that on or around January 18-19, 2022, the facility did not advise any of the parents of COVID positive cases. During the month of January 2022, CCL received UIR’s for positive COVID-19 cases at the CCC for the dates January 7th, January 20th, January 23rd and January 24th, 2022. Per file review, the CCC was notified on Friday, January 7th and Thursday, January 20th of those COVID positive cases. In addition, the CCC was notified on Monday, January 24, 2022 of the January 23rd and January 24th cases. Confidential interviews resulted in conflicting information being provided to LPA. Confidential interviews disclosed that parents were aware of positive COVID-19 case within the CCC but parents were never notified by the CCC; whereas other confidential interviews disclosed that parents were notified by the CCC of the positive COVID-19 cases via email, text, telephone or face-to-face. The Director advised LPAs Hamilton and King that the CCC notified some Room #7 parents on January 7, 2022 via telephone and other parents when they showed up to drop-off their children. On February 16, 2022, LPAs conducted record reviews at the facility and observed that written notification was sent out to parents January 24, 2022 regarding COVID-19 positive case in Classroom #4. The notification was sent Classroom #4 parents and a general notification to all enrolled families.

Regarding the allegation staff handled day care child in a rough manner, it was alleged that S1 is often rough and physical with the children. It was also alleged that S1 grabs the children and sits them down in a rough manner. Confidential interviews again resulted in conflicting information being provided to LPA. A confidential interview revealed that S1 was observed grabbing a child by the shoulders and seating the child roughly; whereas other confidential interviews disclosed that no one has observed S1 or any other teacher in the CCC handle any child in a rough manner. Confidential interviews also revealed to LPAs that none of the parents or children have made the CCC aware of their concern for this type of staff behavior at this CCC.

Based on confidential interviews and record reviews, the allegations that staff is not practicing PPE, licensee did not notify parents of positive COVID cases and S1 handled a day care child in a rough manner, may have occurred, however are not supported or proven by evidence. Therefore, the allegations are unsubstantiated. A copy of this report, appeal rights and Notice of Site Visit were provided to Director.

The Notice of Site Visit was posted by the Director prior to LPAs leaving the facility and the Director was reminded this notice must be posted for 30 days.
