Licensing Program Analyst (LPA) Vicky Williamson conducted a annual random inspection. LPA met with Director, Nesrin Mohammad, and shortly after LPA's arrival Hunan Arshakian, Facility Administrator arrived. There were 14 preschool children present in the front play area of the facility with 1 teacher an the Director. Classroom #2 had 8 toddlers with 2 teachers. The facility operates within licensed capacity and ratio limitations. Children were observed to be under visual supervision. The facility operates Monday - Friday from 6:30 am to 6:00 pm.

The indoor and outdoor of the facility was inspected. The classrooms and restrooms have adequate lighting, heating, and ventilation. All floors appeared to be safe and clean. Furniture, children's cubbies, toys and napping equipment (cots) appeared to be in good condition. Trash can on the playground did not have a cover and contained food and other potential hazardous items. Disinfectants, cleaning solutions and other hazardous items were not stored in locked cabinets. LPA observed a container of bleach and a bottle of Windex in a cabinet that was open in classroom #1 inside of the bathroom above the toilet accessible to children. Medication policies and procedures were reviewed. Menu is posted weekly. All food was inspected and protected from contamination. Food and beverages are stored in covered containers at 45 degrees F or less. Sign in/ sign out sheets were reviewed showing parent/guardian’s signature and time of day recorded. The kitchen and storage areas appeared to be clean. The surface of the outdoor activity space is maintained in a safe condition with sufficient shade. Drinking water is available inside the classrooms and in outdoor play area. There are no bodies of water and weapons present on the premises. The last fire drill was conducted and documented on 10/17/19. The director's office is designated for use by children who are ill. A sample of the children's records were reviewed and include an Admission Agreement in each child’s file. Staff records and transcripts were reviewed to verify teacher qualifications and experience. Opening and closing staff members have current CPR and First Aid certifications. A review of staff records on 11/22/19 indicated that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA and Director reviewed reporting requirements, teacher/child ratio, physical plant, sign in /sign out procedures.

See LIC 809 for deficiencies cited during today's inspection. Facility was provided a copy of the appeal rights form LIC 9058 and the signature on this form acknowledges receipt of these rights.

The Notice of Site Visit (LIC 9213) was provided to be posted at the facility for 30 days. LPA observed form LIC 9213 posted.
