25 | On 11/30/22 at 8:00 AM , LPA Annette Sutherland conducted an unannounced Case Management Inspection due to reported Lead Exceedance levels in the facility's water. LPA met with Site Director, Maria Naranjo. There were 11 children and 7 staff present in the preschool rooms.
Water outlets reported with 5.5 ppb or greater lead exceedance levels as follows:
Kitchen Faucet (28.0 UG/L)
LPA observed "Do not use" sign posted by the sink faucet during visit. Site Director submitted pictures of sink faucett that was taped off and had "out of order" sign taped but has since been replaced and is pending retest. Ms Naranjo stated that they are currently running the water and logging it on a flushing tracking sheet for the next 3 weeks . Ms. Naranjo stated that after 3 weeks the sink faucet will be retested and will not use the sink until the results show the sink faucet is below the exceedance lead level.

LPA Annette Sutherland informed facility representative Maria Naranjo that this report dated 11/30/22 documents 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Annette Sutherland informed the facility representative Maria Naranjo to provide a copy of this licensing report dated 11/30/22 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. |
