32 | According the RP, the daily report that was sent home on 1/14/2020 indicates the child was fed with Formula instead of the breastmilk that was provided. During an interview the Director stated that the child was actually given the appropriate food (breastmilk), and this may have simply been a documentation error. All other recorded documentation from 1/13/2020 through 2/4/2020 indicate "breastmilk" was offered. Although a similar event was recorded, It cannot be proven that this specific allegation occurred after that time.
Throughout the Tele-visit, LPA's observed an implementation of a color coding system used to assist staff with identifying the individual needs of the infant children. Each infant child has a label that includes their photo, feeding plan, and is color coded, which correlates to their clipboard that has their infant needs and services plan. It is also duplicated on a visible white board, at eye level for staff, which indicates feeding, diaper changing, and medication (if used). The refrigerator located in the classroom that stores the food also has color coded tape on the door to indicate formula/breastmilk and is in conjunction with bottles inside of the refrigerator.
During the tele-visit, LPA's observed children’s Medication stored in each child's cubby, labeled and inaccessible to children. During a previous Interview with Director and staff, It was confirmed that a teacher did apply a diaper cream, on child#1, however, there is not sufficient evidence to support if staff applied diaper cream in accordance with the product label.
During interviews with Director and staff it was stated that the infant classroom is cleaned and sanitized daily. They added that the tables and toys are sanitized throughout the day. The director has additionally ordered a Swiffer-sweeper to assist with daily cleaning of the floors in the infant classroom, to limit the risk of spreading communicable disease or virus. During the tele-visit, the assistant Director also mentioned that due to Covid, daily cleaning and sanitizing has increased, a log is now required, and staff are assigned various cleaning tasks.
Additionally, it was expressed that a child's nap-time was interrupted for feeding. All events are recorded on a tablet, through a program, called "tadpole" which indicates the start and stop times of naps, meal times, and diaper changes. However, during Interview with the director, it was stated that documentation error can occur if for instance a child wakes up and is fed, if the staff does not stop the time prior to feeding the child. Although documentation is important, the priority is to meet the immediate needs of the children in care, and using this electronic system there is room for staff error. Continued on LIC9099-C |
