25 | On 12/29/2022 @ 11:15 a.m., Licensing Program Analyst (LPA), Joelle Redding, made an unannounced visit to issue a citation for deficiencies discovered during prior visits.
Based on observation, the school's transportation van is an 11 passenger van (including the driver), requiring a special endorsement to operate. Although a recent attempt was made to reduce the capacity of the van to 8 by removing the rear seat, this does not change that the van is manufactured to carry 11 passengers regardless. Staff driving the van to pick up and drop off children, do not have the required endorsement. Therefore, a Type A citation will be issued on the accompanying LIC 809D.
LPA , Joelle Redding, informed Director Linda Mendez that this report, dated 12/29/22, documents a Type A citation(s) and shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
LPA, Joelle Redding, also informed Director Linda Mendez to provide a copy of this licensing report, dated 12/29/22, that documents a Type A citation, to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. LIC 9224 was provided to the facility during this visit.
NOTICE OF SITE VISIT WAS GIVEN AND WILL REMAIN POSTED FOR 30 DAYS. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
