On 4/18/2022 at 8:15am, Licensing Program Analyst (LPA), Martha Malane conducted an unannounced inspection for the purpose of delivering findings for the above listed allegations. Upon arrival, LPA met with Director, Colleen Teran and disclosed the purpose of the inspection. LPA proceeded to tour the facility and observed 28 children with five (5) staff members.

On 1/20/2022, Community Care Licensing (CCL) received a complaint alleging child sustained an unexplained injury while in care and child's diapering needs were not met.

During the course of the investigation, interviews were conducted with outside agencies, staff and daycare parents and medical reports and records were obtained and reviewed.

Based on information obtained during the investigation, the child did sustain an injury and diaper rash, however it could not be determined where or how the injury or rash were sustained.

Due to conflicting information and a lack of supporting evidence obtained throughout the course of the investigation, the allegations are determined to be unsubstantiated. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

No deficiencies cited during today's inspection.

A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

An exit interview was conducted with Director, Colleen Teran.
