13 | THIS IS AN AMENDED VERSION OF A PREVIOUSLY WRITTEN REPORT. On 6/14/24 at 9:30am, Licensing Program Analyst (LPA) Adrian Castellon conducted an unannounced inspection to deliver complaint findings for the allegation listed above. LPA met with Center Director Denise Matthews and discussed the purpose of the inspection and toured the facility. LPA conducted staff and children's interviews on this date. During the course on the investigation, LPA conducted staff, parent and children interviews. LPA conducted three unannounced inspections. Based on interviews conducted and documents obtained, the facility was out of ratio on at least two occasions; the facility provided care and supervision for children under two years old and over five years old; and on at least one occasion, staff removed food/drink from a child’s lunchbox which consumed by staff and/or other children. As such, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Violations of the California Code of Regulations, Tittle 22, are being cited on the attached LIC9099D. Three Type B citations issued. Please see LIC9099D. Exit interview was conducted, report reviewed, and Appeal Rights discussed with licensee. A Notice of Site Visit was given and must remain posted on, or immediately next to, interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
