13 | On 12/9/22 at 10:00am, Licensing Program Analyst (LPA) Adrian Castellon conducted an unannounced inspection to deliver complaint findings for the above allegations. LPA Castellon met with owner/director Denise Matthews and discussed the purpose of the inspection. It was alleged that: Staff did not ensure that day care child was free from being bullied by another child while in care and that Staff yelled at day care child while in care. During the course of the investigation, four unannounced inspections were conducted. Interviews were conducted with day-care parents, day-care children and facility staff. LPA obtained texts pertaining to the incident. After several attempted phone calls, LPA Castellon could not interview the reporting party. Child involved in the allegation no longer attended the facility. LPA Castellon could not corroborate the allegations through interviews conducted or own observations.
Due to conflicting statements obtained during the course of the investigation, the above allegation is deemed to be UNSUBSTANTIATED meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged allegation occurred. A copy of today's report, Notice of Site Visit and appeals rights given to the licensee. An exit interview was conducted with facility director stated that she understood. Notice of Site Visit should be posted for 30 days from today's date. |
