13 | On 6/14/24 at 9:30am, Licensing Program Analyst (LPA) Adrian Castellon conducted an unannounced inspection to deliver complaint findings for the allegation listed above. LPA met with Center Director Denise Matthews and discussed the purpose of the inspection and toured the facility. LPA conducted staff and children's interviews on this date. During the course on the investigation, LPA conducted staff, parent and children interviews. LPA conducted three unannounced inspections.
Based on the interviews conducted and documents obtained (time sheets), the allegations are substantiated. As such, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Violations of the California Code of Regulations, Tittle 22, are being cited on the attached LIC9099D. Two Type B citations issued. Please see LIC9099D.
Exit interview was conducted, report reviewed, and Appeal Rights discussed with licensee. A Notice of Site Visit was given and must remain posted on, or immediately next to, interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
