On 12/27/2024, at 12:30 pm., Licensing Program Analysts (LPAs) Danielle Anderson and Michelle Hood conducted an unannounced case management inspection regarding a self - reported incident wherein a 3-year-old child was found unsupervised on the playground, while the children and teachers were inside the facility. LPAs met with Site Supervisor, Marissa Baca. LPAs discussed the purpose of the inspection and was led inside the facility.

The following ratios were observed: Classroom 1 had 17 napping children present with 1 aide, Classroom 3 had 15 napping children present with one aide, and Classroom 2 & 6 were closed.

Site Supervisor stated on 12/19/2024 there were 9 pre-school children with two staff from classroom #1 on the playground. LPAs and site supervisor reviewed the video footage from 12/19/2024. A review of video footage disclosed the child on the playground playing under the slide alone from 4:30 pm to 4:37 pm. The staff and other students from classroom #1 went inside at 4:30 pm. Staff conducted a headcount which did not match. Even after the head count not matching the staff changed the number on the board as if the child was accounted for. At 4:37pm, the child was observed on the playground by a teacher inside the classroom. At that point, a staff member from classroom 1, went outside and retrieved the child from the playground.

Based on video footage, documentation, and interviews, one Type A deficiency of California Code of Regulations, Title 22, Division 12, Chapter 1, is being cited on the attached LIC 809D.

LPA Anderson informed Site Supervisor, Marissa Baca that this report dated 12/27/2024 documents one Type A citation, one Type B which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA informed site supervisor Marissa Baca to provide a copy of this licensing report dated 12/27/2024 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. LPA Anderson provided Site Supervisor, Marissa Baca with form LIC 9224.


The site Supervisor was provided appeal rights (LIC 9058) and their signature on this form acknowledges
receipt of these rights. Notice of Site Visit (LIC 9213) was provided to be posted at the facility for 30 days.
LPA observed form LIC 9213 posted. An exit interview was conducted.

The Assistant Director is going to conduct a zoning and supervision training, staff will watch the Supervising Children in Child Care Centers on CCLD website, Active Supervision and Outside Play -

Based on documentation, video footage and interviews, the facility staff did not comply with the section cited above, as a child was left unattended on the playground for approximately 6 minutes before staff noticed the child on the playground. This poses an immediate health, safety, and personal rights risk to persons in care.

Site Supervisor will create a plan on how they will ensure a qualified teacher is present, at all times when children are in care. Site supervisor will conduct a meeting and provide LPA with a copy of the agenda which includes aides only working under the supervision of a teacher.
