25 | On 12/27/2024, at 12:30 pm., Licensing Program Analysts (LPAs) Danielle Anderson and Michelle Hood conducted an unannounced case management inspection regarding a self - reported incident wherein a 3-year-old child was found unsupervised on the playground, while the children and teachers were inside the facility. LPAs met with Site Supervisor, Marissa Baca. LPAs discussed the purpose of the inspection and was led inside the facility.
The following ratios were observed: Classroom 1 had 17 napping children present with 1 aide, Classroom 3 had 15 napping children present with one aide, and Classroom 2 & 6 were closed.
Site Supervisor stated on 12/19/2024 there were 9 pre-school children with two staff from classroom #1 on the playground. LPAs and site supervisor reviewed the video footage from 12/19/2024. A review of video footage disclosed the child on the playground playing under the slide alone from 4:30 pm to 4:37 pm. The staff and other students from classroom #1 went inside at 4:30 pm. Staff conducted a headcount which did not match. Even after the head count not matching the staff changed the number on the board as if the child was accounted for. At 4:37pm, the child was observed on the playground by a teacher inside the classroom. At that point, a staff member from classroom 1, went outside and retrieved the child from the playground.
Based on video footage, documentation, and interviews, one Type A deficiency of California Code of Regulations, Title 22, Division 12, Chapter 1, is being cited on the attached LIC 809D.
