On 3/5/2025, at 8:55am, Licensing Program Analyst (LPA) Vicky Williamson conducted an unannounced annual random inspection. LPA met with Assistant Director, Patricia Jauregui and disclosed the purpose of the inspection. LPA accompanied by Assistant Director toured the indoor of the facility. At 9:30am, Facility Representative, Renece Mc Morris arrived to the facility and accompanied LPA on the tour of the indoor and outdoor of the facility. This is a full day program which operates all year round. Days and hours of operation are Monday through Friday from 6:00am to 6:00pm. There is currently one infant room that is separated into a mobile infants and non-mobile infants. The following ratios were observed:

Mobile Infant Room – 12 infants with two (2) teachers and one (1) aide.
Non – Mobile Infant Room - 4 infants with two (2) teachers.

The facility has sufficient age-appropriate furniture, and equipment including cribs, cots or mats, changing tables and feeding chairs. Potty chairs, toilets and hand-washing equipment are in safe and sanitary operating condition. Changing table is within arm’s reach of a sink. Floors in the facility are clean and safe. Disinfectants, cleaning solutions, medications and other hazardous items were accessible to children during the time of inspection. LPA observed the bathroom door inside of the classroom on the Mobile Infant side of the room, open and the safety gate was not latched. LPA observed the following: Clorox spray, all purpose cleaner and bleach underneath the sink in a cabinet; two rolls of trash bags in a drawer attached to the sink, two containers of Clorox disinfecting wipes in a corner next to the toilet; a Swiffer mop with the liquid cleaner loaded in the mop located behind the bathroom door. Facility Representative removed all items during the time of inspection.

All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents. Solid waste storage containers have tight fitting covers and are in good repair. Facility has smoke and carbon monoxide detectors that meet statutory requirements. The last fire drill was conducted and documented on 1/24/2025 and earthquake drill on 2/12/2025. The outdoor activity space for infants is physically separate; however LPA observed 11 preschool children playing on the infant playground. LPA observed that a portion of the infant playground has been added and the Department was not notified of the change. Facility Representative verified with Licensee Representative that the infant playground is exclusive for infant's use only. The surface of the outdoor activity space is maintained in a safe condition and free of hazards. Outdoor play area is fenced with sufficient material for cushioning. Area has canopies used for shade. Filtered drinking water is available both indoors and outdoors by use of water container and children's water bottles. Facility provides snack and lunch. Menus are posted weekly.

The infant room has a refrigerator used for infant food storage. LPA observed a bottle containing breast milk in the refrigerator for Child #1 (C1) that did not include a name or date on the bottle. Facility Representative is reminded that food storage and bottles should include the infant's name and date. Formula is provided by the facility.

Facility Representative stated that there are no swimming pools or other bodies of water on the premises. Facility Representative stated that there are no firearms or ammunition allowed or stored on the premises.

Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Capacity and limitations as specified on the license are being maintained. Children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than four (4) infants in care.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. The name of the facility representative and the fully qualified teacher designated to act in the facility representative’s absence has been reported to the Department. LPA reviewed facility sign in /sign out sheets and observed they were incomplete with the arrival time for Child #2 and Child #3.

Facility Representative was reminded that the person who signs the child in and out of the facility shall use their full legal signature and record the time of day.

LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. Documentation for Infant 15 - minute sleep checks were reviewed and are being maintained. Individual Infant Sleeping Plans are completed and on file for each infant up to 12 months of age. Infant Needs and Service Plans were reviewed and were not updated quarterly for Child #4 (C4), Child #5 (C5), Child #6 (C6), Child #7 (C7) and Child #8 (C8).


LPA reviewed a sample of staff files and observed files were complete with health screenings, immunization records for influenza, pertussis and measles; however current mandated reporter training certification for Staff #1 (S1) was not available for review.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Per California Code of Regulations, Title 22, Division 12, Chapter 1, six (6) Type B deficiencies are being cited on the attached LIC 809D.



One Technical Violation was issued.

A Notice of Site Visit (LIC 9213) was given to Facility Representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. LPA observed LIC 9213 was posted. Appeal Rights (LIC 9058) was provided. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted and the report was reviewed with the Facility Representative Renece Mc Morris.
