Licensing Program Analyst (LPA) Jennifer Lott conducted a complaint investigation visit to deliver findings for the above allegation. LPA was greeted at the front of the facility by Director, Valerie Rodriguez and was granted entry after identifying herself and disclosing the reason for her visit.

The Department’s investigation consisted of record reviews and interviews. On or about March 09, 2023, it is alleged that child #1 (C1) sustained an unexplained injury while in care. Interviews revealed that two children, who are both 2 years old, were playing together. Child #2 (C2) was reaching for a toy that C1 was playing with and while reaching to get the toy, accidentally scratched C1 on the surface of their cheek with the nail on their hand. Neither child cried, and both continued playing together.

Parent was notified of the scratch, signed the incident report and acknowledged the incident on 03/09/23.

This agency has investigated the complaint alleging child in care sustained unexplained injury. The Department has found that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. An exit interview was conducted and report was reviewed with Director Rodriguez. A notice of site visit was given and must remain posted for 30 days.
