1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 04/25/2022 at 10:45 a.m., Licensing Program Analyst (LPA), Rajani Goudreau conducted an unannounced Annual Inspection and to inspect an outdoor playground addition for the 12 additional school age children. upon arrival LPA met with Director, Dorothy Evans. LPA disclosed the purpose of the inspection and toured the facility indoors and outdoors. There are currently six classrooms in operation. The following ratios were observed: #103: 10 children/two staff members, #104: 14 children/two staff members, #109: 9 children/one staff member and #110: 18 children/two staff members. There were 51 children in care with seven staff members present. This is a full day program that operates year around. Days and hours of operation are Monday through Friday from 7:00 a.m. to 6:00 p.m. There were no bodies of water observed on the premises, per observation. There are no firearms or ammunition on the premises, per Director. Disinfectants, cleaning solutions, medication and other hazardous items were made inaccessible, per observation. No poisons were observed during the inspection. LPA informed Director, all poisons shall be kept in a locked storage area. Furniture and equipment in the classrooms were observed to be in good condition, free of sharp, loose or pointed parts. The playground equipment and play materials were observed to be in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space was observed to be maintained in a safe condition and is free of hazards. All toilets and hand washing facilities were observed to be in safe and sanitary operating condition. The floors in the facility were observed to clean and safe. Drinking water is available both indoors and outdoors by the use of refillable water containers and reusable cups brought from home.

Areas around high climbing equipment were observed to have sufficient cushioning material by the use of wood chips to absorb falls. Shade was observed in the outdoor activity space by the use of a large shade kite and a canopy. Based on observation, the facility has a functioning carbon monoxide detector that meet statutory requirements.

A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained, per observation. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility, based on file review. Manual sign in/out sheets utilized. Children were observed to be under supervision, including visual supervision. Based on observation, the facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles. Menus are posted at least one week in advance where an authorized representative can view them. Snack is provided by the facility.

Director is requesting for the 12 school age children to utilize the Lighthouse Christian Academy’s outdoor play area. In order, to allow the school age children to utilize a larger space during outdoor activities. The academy’s playground is approximately 290 steps from the school age classroom.

According to the Director, the academy’s playground will only be utilized by the 12 additional school age children during the summer when the academy is not in session. During the times, when the 12 additional school age children are not utilizing the academy’s playground the children will continue to share the preschool playground on a scheduled basis as permitted under the approved waiver dated August 4, 2017. The director has submitted the proposed schedules for when the 12 additional school age children will utilize the academy’s playground, transition to and from the outdoor play area and supervision to be maintained. In addition, a calender reflecting when the academy is not in session. Per the tour of the facility today, the proposed school age playground outdoor activity space maintains a shaded rest area for the children by the use of a removable canopy and trees. The surface of the additional school age outdoor space is maintained in safe conditions and free of hazards. The facility will provide drinking water to the children through the use of refillable water containers and assigned water bottles. The facility maintains cushioning material through the use of rubber material. The outdoor area is enclosed by a fence that is at least four feet high. All play equipment is age appropriate. There is more than sufficient space in the outdoor play area to accommodate the licensed capacity. The playground area is in compliance with standards established in CCR, Title 22, Division 12, Chapter 1, for Child Care Centers.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility provides Incidental Medical Services (IMS). LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
