25 | On January 21, 2021 at 4:28 PM Licensing Program Analyst (LPA) Adrian Mangina conducted an unannounced Case Management Inspection via Facetime in reference to a report that the facility is not informing parents of their children’s possible exposure to Covid-19. LPA met with Director Ana Lopez and proceeded to tour the facility. Present today were 29 children and 5 staff members. Appropriate ratios were observed. All staff members have the required background clearances and are associated to the facility.
Room/Age/number of children/staff members
Room 1 – Bumblebees, there were 12 4 and 5 years old children supervised by one teacher, Mariah Osman
Room 2 – Butterflies: there were 17 3 and 4 year old children supervised by two staff, Kayla Arena and Brittany Thomas
Room 3 – Ladybugs, this classroom was closed due to all teacher and children are quarantined from 1/12/21 through 1/21/21
The Department received an anonymous report that a parent tested positive for Covid-19 and that confidentiality was not maintained as others overheard the conversation reporting the diagnosis. The report also alleged that the facility did not notify parents or staff as required. It is also alleged that there are children who have tested positive for covid-19 and the facility has neither informed parents of the exposure, have not closed the classroom and have not sanitized as required by Health Department guidelines.
The Director was notified on 1/11/21 that Child Kobe Queen in Landybugs tested for Covid-19 on 1/7/21. The test results came back on 1/10/21 as positive. His last day at the school was 1/6/21. The child’s father told the mother that he was not feeling well on 1/7/21 and that he had covid symptoms. The father was tested (date unknown). The child’s mother decided to get the child tested as a precaution as the child had a visit with the father on 1/5/21.
