On 5/25/23 at 1:30 PM, Licensing Program Analysts (LPAs) Keturah Lane and Gerald Poindexter visited the facility to conduct an annual inspection. Upon arrival LPA met with Director Ana Lopez and proceeded to tour the facility. Census was as follows:

Classroom Monkeys: 22 children with staff members: Elizabeth Carpio and Ebone Singleton (11 children were inside & 11 children were outside on playground)
Classroom Ladybugs: 21 children with staff members: Amber Mallobox-MacCartee and Estrella Andrade
Classroom Bumblebees: 22 children with staff members: Maria Cabrera and Maria Ossmen-Conway
Butterflies: 16 children with staff members Daniela Navarrete and Briana Gomez
Classroom Dragonflies: 11 children with staff member Martha Villalba (Ms. Patty)

Appropriate ratios and capacity were observed. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation. Floors appear to be clean and safe. Drinking water is readily accessible. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a kitchen which is clean and free of hazards. Menu is posted. Adequate food is available for meals. Cleaning supplies are kept separate from food and are inaccessible to children. Storage containers for solid waste have tight-fitting covers and are kept in good repair. Storage areas for poisons are locked. Director stated there are no firearms or other weapons on the premises. All foods/beverages capable of rapid spoiling are stored in covered containers at 45 F or less. The facility appears to be free of insects and rodents. There is an operational carbon monoxide detector at the facility. (continued on LIC809-C...)

Outdoor play area is a fenced playground with sufficient material for cushioning. There are no bodies of water at this facility. Climbing structures, swings and slides are securely fixed to the ground. Area has canopies used for shade. Equipment is age appropriate. Area has drinking water readily accessible and grounds are free of debris or potential hazards.

LPA reviewed medication storage. LPA observed 4 children with inhalers that did not have prescription label attached. One of the children also had an epi-pen that was expired and without a prescription label. Staff member S1 did not have health screening documentation or negative TB on file. At least one staff member has current CPR and First Aid certifications. Staff members S2 and S3 did not have mandated reporter training on file. All staff had required immunizations. Four staff members (S1, S3, S4 and S5) did not meet teacher qualifications and did not have sufficient units. There was not a qualified teacher in room Ladybugs or Butterflies at time of LPA's inspection. Sign ins are through Brightwheel app. Children’s records contain admission agreements and medical assessment. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
