32 | The allegation that “Lack of supervision resulting in daycare child to self-harm” is verifiable. The LPA’s investigation yielded evidence to corroborate the allegation and its associated details. Based on information obtained during LPA observation of the facility, review of facility records, and interviews with staff and parents, it is determined that the allegation is valid.
Per information obtained during interviews, it was determined that staff did not intervene when a child exhibited harmful behaviors, resulting in injury to the child. The allegation is valid because a preponderance of the evidence has been met, therefore, the above allegation is found to be SUBSTANTIATED.
