25 | On March 17, 2022 at 11:50 a.m. Licensing Program Analyst (LPA) Leilani Curtis and Licensing Program Manager (LPM) Tashima Daniel conducted an unannounced case management inspection. Upon arrival LPA and LPM met with Director Celene Marin and proceeded to tour the facility. There were 15 children with 5 staff members present. Appropriate ratio/capacity were observed. Staff members have the required background clearances and are associated to the facility.
On 1/12/22 Director of Operations Maisi Nguyen advised LPA that there were several daycare children and staff in the facility that were diagnosed positive for Covid-19 during the months of November 2021, December 2021 and January 2022. Ms. Nguyen stated that she did not report the positive cases to Community Care Licensing because she was not aware of the reporting requirement. She also stated that the LIC624, Unusual Incident form was not submitted within 7 days of the incident (positive diagnosis). On 1/18/22 LPA was advised that at least 6 people associated with the facility had tested positive for Covid-19. Director Celene Marin also states that she was not aware that the positive Covid-19 cases were required to be reported to Community Care Licensing.

An exit interview was conducted with the director and appeal rights (LIC 9058 1/16) were discussed. The director’s signature on this form acknowledges receipt of these rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA and LPM observed the director post notice of site visit. |
