25 | On October 14, 2021 at 12:36 p.m. Licensing Program Analyst, Leilani Curtis, conducted an unannounced inspection to follow up on a self-reported incident that occurred on October 5, 2021 wherein a child (C1) was playing in front of the play structure when he slipped and fell. Upon arrival LPA met with teacher Madeline Styer and proceeded to tour the facility. There were 24 children with 2 staff members present. Appropriate ratio/capacity were observed. Staff members have the required background clearances and are associated to the facility. At 1:10 p.m. the District Director, Jennifer Orr arrived at the facility.
LPA interviewed Staff #1 (S1) and the District Director (D1). On 10/05/21 at approximately 12:40 p.m. C1 was playing in front of the rope ladder on the play structure when he slipped and fell through the bottom square of the ladder and landed on the wood chips below the structure. The ladder is made of rope and the play structure is age appropriate. There are sufficient wood chips below the structure to absorb the impact of a fall. The bottom rope that the child fell over is approximately 1 ft, 22 inches from the ground. At the time of the incident there were 3 children and 1 staff member present on the playground. Proper ratio and supervision were in place. As soon as the incident occurred S1 comforted the child and looked for injuries. The child did not indicate that he was injured at the time. The child’s nanny who arrived at the facility immediately after the accident was notified of the incident. The child went to the doctor where he was diagnosed with a fracture to the right wrist. His right arm was placed into a cast. The child returned to the facility on October 7, 2021 and remains in care. The District Director states that after the incident the playground was inspected to make sure it was safe and the site director followed up with the child's parents. The facility responded appropriately and reported timely.
No deficiencies are cited.
An exit interview was conducted with the district director and appeal rights (LIC 9058 1/16) were discussed. The district director’s signature on this form acknowledges receipt of these rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed the district director post notice of site visit. |
