On 10/22/2021 @ 11:10AM, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced Plan of Correction (POC) inspection in reference to the deficiency cited on 10/14/2021. The purpose of this inspection is to observe the correction.

Met and toured the classrooms and outdoor play area with Bree Zuelzke, Site Director. Observed present today were 26 children in the following classrooms:

Room #5 with 7 infants and staff Silvia Gonzales & Jasmine Villaescuza
Room #7 with 7 infants and staff Jessica Madrigal & Chaunielle Robinson
Room #9 with 8 infants and staff Claudia Ramirez, Giselle Fernandez & Charosa Archuleta
Room #11 with 4 infants and staff Kelly Bryan & Rene Gonzales

All staff were observed wearing masks inside the classrooms.

NO DEFICIENCY WAS CITED TODAY.

Exit interview was conducted with Bree Zuelzke. Appeal rights were discussed. A copy of this report and appeal rights were provided. Notice of Site visit was observed posted. This notice shall remain posted for 30 days.
