25 | On 12/6/23 at 2:15 PM Licensing Program Analyst (LPA) Adrian Mangina conducted an unannounced visit to follow up on a self-reported incident that occurred on 11/8/23 wherein Child #1 hurt child #2 during an argument, causing Child 2 to require medical care. LPA met with Director Jodie Debus.
During the visit LPA interviewed Director and witnesses to the incident. LPA interviewed the witnesses who on the playground during the incident. Neither directly saw the incident as it happened quickly when each had attention on other children but they did hear child 2 cry out. Staff intervened and asked child 2 if they were hurt and they said they were scared not hurt. Later that day child was seen doing handstands and flips on the bars. The next day the child was seen at urgent care for pain and strain, but returned to care the next week. Director stated that she spoke to staff and reminded them to better supervise while outside and the facility has since limited the area of the outside playspace to ensure visual supervision of all children at all times.
At the time of the incident there were 23 children and two staff present. Supervision was in place, ratios were met, the facility responded appropriately and reported timely. No deficiencies are cited.
Exit interview conducted and report was reviewed with the facility representative Josie Debus. A notice of site visit was given and must remain posted for 30 days. |
