On 08/01/2023 at 2:01pm, Licensing Program Analyst (LPA) Selina Siao conducted an annual inspection. Upon arrival, LPA met with Office Administrator Gricelda Olvera and proceeded to tour the facility until Director Cristy Nova was available. The following ratios were observed: Cuddle Bear (Older Infant) had 4 children supervised by teacher Roya Seifizarei and teacher aide Erfaneh Teimouri, Baby Bear had 6 infants supervised by teacher Shuri Li and Mozhgan Eskandarloo. All required notices, forms and licenses were posted. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and drinking water available for children. Storage cubbies are readily available, and room accommodates class size. Napping equipment consists of cribs and mats which are kept in classroom and each mat has its own sheet or covering. Food service area consists of a kitchen which is clean and free of hazards. Menu is posted. Adequate food is available for snacks. Cleaning supplies are kept out of children. Outdoor play area is located by the Baby Bear room and Tigers Room and the playground is fenced with turf. Both areas are located under the building shade area. Equipment is age appropriate and separated by age groups, infant, toddlers and preschool. Drinking water is available for the children to refill their zippy cups. LPA reviewed sign in sheets, first aid supplies and reviewed medication policy and storage. Isolation area is the Director's office. Personnel and a sample of client records were reviewed. Not all staff members have the required immunizations. All staff member completed the online mandated reporter training and has a valid certificate. Reporting requirements was also reviewed. All personnel have required criminal record and child abuse index clearances or exemptions. LPA reviewed Emergency Disaster Plan and last fire drill was conducted on 12/13/2022. Facility has an operating carbon monoxide detector located in the Tigers room. Facility shall have at least one staff member that has a valid EMSA approved CPR/FA when children are in care. Isolation area is the Director's office.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was provided with PIN 23-09-CCP Extreme Heat Hazard Awareness Focusing on Infants and Young Children and Safe Sleep Regulations, Effects of Lead Exposure and reporting responsibilities were discussed. The ABC’S of Safe Sleep: Sleep is Safest: Alone, on their Back in an empty Crib on a firm mattress.

Child Care Providers can now sign up for Quarterly Updates and PINS through the DSS website at https://cdss.ca.gov/inforesources/community-care-licensing/subscribe. LPA discussed California Megan's Law with provider and advised her to go on the website at www.meganslaw.ca.gov.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Please complete the survey and share your inspection experience. LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

All unusual incident reports shall be submitted to Licensing office via email at SDIncidentReports@dss.ca.gov or via fax at (619)767-2203.

Refer to the next page LIC 809D for deficiency citation. An exit interview was conducted, and appeal rights were provided.
