13 | On 01/21/2026 at 10:00 AM, Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced complaint inspection to deliver findings on the above allegation. LPA met with Director, Alexa Denton and informed her of the reason for the visit. There were15 children present with 4 staff members at the time of this inspection.
During the course of the investigation, LPA conducted two unannounced inspections, interviewed Director, Assistant Director, staff members, and daycare parents, photographed the Infant playground and reviewed facility records, including incident reports involving playground injuries dated 07/01/25-10/31/2025.
During interviews Director denied allegation stating that occasional incidents of infants falls during outdoor play are to be expected for the age group, as many of the infants are still learning to walk. During interviews, 4 out of 7 staff stated that there were an excessive number of infant falling injuries on the infant playground. During LPA review, it was revealed that there were seven (7) incidents of infants falling on the playground in October 2025.
