On 01/21/2026 at 10:00 AM, Licensing Program Analyst (LPA) Dana Stevens conducted an unannounced complaint inspection to deliver findings on the above allegation. LPA met with Director, Alexa Denton and informed her of the reason for the visit. There were15 children present with 4 staff members at the time of this inspection.

During the course of the investigation, LPA conducted two unannounced inspections, interviewed Director, Assistant Director, staff members, and daycare parents, photographed the Infant playground and reviewed facility records, including incident reports involving playground injuries dated 07/01/25-10/31/2025.

During interviews Director denied allegation stating that occasional incidents of infants falls during outdoor play are to be expected for the age group, as many of the infants are still learning to walk. During interviews, 4 out of 7 staff stated that there were an excessive number of infant falling injuries on the infant playground. During LPA review, it was revealed that there were seven (7) incidents of infants falling on the playground in October 2025.

Based on the information obtained in interviews and record review the preponderance of evidence standard has been met and the allegation of Licensee did not ensure the safety of infants on uneven outdoor surface of the playground is found to be Substantiated.

California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 9099-D.

Upon Receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.

Exit interview conducted and report was reviewed with Director, Alexa Denton.
Appeal Rights (LIC 9058) were provided. Notice of Site Visit (LIC 9213) must remain posted for 30 days.
