On 02/19/21 at 08:55 a.m., Licensing Program Analyst (LPA), Rajani Goudreau arrived at the facility to conduct a Case Management – Licensee Initiated inspection.  Upon arrival LPA, met with the Director, Amanda Lundy and proceeded to tour the facility. There were seven infants in care with three staff members at time of visit. The facility is within ratio/capacity limitations. The facility is requesting a capacity increase of 24 infants; 0 through 2 years old and adding classroom #130 to the license in order to accommodate the requested capacity. Facility operates Monday through Friday from 7:00 a.m. to 6:00 p.m.

Rooms #130 and #140 were inspected and measured during this inspection and measured the following:

Room #130: measured a total of 528.29 square feet

Room #140: measured a total of 423 square feet

The rooms measured a total of 951.29 square feet which is sufficient to accommodate 27 infants. However, facility is requesting a capacity of 24 infants. The furniture and play equipment in the rooms are safe, age-appropriate and in good repair.  The rooms have adequate heating, lighting and ventilation, are clean and orderly, and are free of hazards. The rooms have a separate area for napping, including cribs. The napping area was not included in the indoor square footage calculation. The infant changing tables are padded with washable vinyl at least one inch thick with sides raised at least 3 inches, per regulation and are within arm’s reach of a sink. LPA informed the director the sink near the diaper changing table shall not be used for food preparation. There is two restrooms located in rooms #130 and #140 each containing one sink and one toilet. LPA discussed with the director the regulations regarding the infant needs and services plan. All storage containers and trashes have tight fitting covers and are in good repair. The highchairs for infants are in good repair and made of a washable, moisture resistant material. Drinking water is readily available inside the classroom via refillable water containers and Sippy cups brought from home. See LIC809-C continuation page...

The isolation areas are located in classrooms #130 and #140. LPA informed the director, the isolation area shall be equipped with a crib, cot, mat for each ill infant. Also, any infant in the isolation area shall be under constant visual observation by a staff member. There is a carbon monoxide detector in the room and a first aid kit with required supplies. Required documents are posted .

LPA reviewed the outdoor play equipment and toys to be age appropriate. Outdoor play yard for the infants was previously measured at 2,168 square feet, which is sufficient to accommodate 28 infants. The outdoor play area is fenced with turf cushioning around the climbing structures and is separate from the preschool playground. The carbon monoxide detector is operational, per LPA observation. Drinking water is readily available via refillable water containers. There is adequate shade in the outdoor play yard via trees. Proper supervision and placement of staff during outdoor activities was advised to ensure the safety of children in care at all times.

This facility provides Incidental Medical Services - IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children, personnel and administrative records. A fire clearance for 24 infants was granted on February 12, 2021.

No deficiencies issued during today's visit. LPA conducted an exit interview with the director. LPA discussed and provided the following: LIC809, LIC809-C, LIC9213-Notice of Site Visit and Appeal Rights. A capacity increase for 24 infants; ages 0 through 2 years old in rooms #130 and #140 shall be granted after final file review. Notice of Site Visit shall be posted for 30 days from today's date.
