Licensing Program Analysts (LPAs) Selina Siao and Leilani Curtis conducted a case management visit to follow-up on two incidents involving a child that sustained an injury and obtained medical treatment on 5/7/2019. Another incident involved two children that were left inside the classroom on 5/8/2019. LPAs met with Director Veronica Rosillo about 20 minutes later after she returned back to the facility. Facility was toured, and the following ratios were observed in the three classrooms; Room 1/Pablo Picasso had 19 children resting on cots supervised by teacher Ramona Carrasco and teacher aide Jackie Ramirez. Room 2/Diego Rivera had 18 children resting on cots supervised by teacher Enma Flores and teacher aide Lolita Aguilar aka Maria. Teacher Perla Argumedo returned from lunch during the inspection. Room 3/Leonardo Da Vinci had 23 children resting on cots supervised by teacher Brenda Puentes. LPAs observed children involved in indoor and outdoor activities during the inspection. Appropriate ratios were maintained. Staff present have the required criminal background clearances and are associated to the facility.

Both incidents were self-reported by the facility and both written reports were received in the Licensing office within the 7 days required period. The facility was within ratio in both incidents. The first incident occurred on 05/07/2019 at the playground. Two children were playing ball at the playground. One child fell on the floor and the second child tried to skip over the child that was on the floor and he hit his head on the wooden gazebo by accident. The second child sustained a cut on his head and obtained medical treatment. During the incident there were 16 children.

supervised by two staff members. The child return to care the next day without restrictions. Interviews were conducted with the two staff members and it appears that it was an accident. Director stated that the facility plans on cutting down the side of the gazebo to ensure children's safety. Staff administered first aid and notifying the parent. Teacher discussed classroom safety rules with children in care.

The second incident occurred on 05/08/19 in the classroom. Two children were left unattended inside the classroom. The rest of the children in class had transitioned to the restroom with two staff members. LPAs reviewed the video footage of the incident, conducted interviews with the two staff members and spoke with the parent that found the two children in the classroom. Based on information obtained, the children were left unattended in the classroom for about two minutes before the NHA staff member went into the classroom and found out about the unattended children. The twelve children and the two staff members returned from the restroom to the classroom about four minutes later per video footage.

Please see the next page LIC 809D for deficiencies citations. An exit interview was conducted, and appeal rights were provided. A notice of site visit was provided and to be posted at the facility for 30 days.

The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1).Supervision shall include visual observation.
