25 | Licensing Program Analysts (LPAs) Selina Siao and Gloria Gonzalez conducted an unannounced case management inspection and to approve a new area next to the kitchen. Upon arrival, LPAs met with current Site Supervisor Tomeka Williams and conducted a tour of the facility.
The area next to the kitchen is set up with tables and chairs and it's a semi enclosed area for children to use. No hazardous items are observed and can be use for children effective today.
The following ratios were observed: 17 children were observed in room #1 (Leonardo) supervised by substitute teacher Dania Hernandez and teacher assistant Ana Valenzuela. 18 children were observed in room #2 (Picasso) supervised by teacher aides Maria Dolores Aguilar and Maria Lopez. 22 children were observed in room #3 (Diego) supervised by teacher Ramona Carrasco and teacher aides Abeer Alhassan and Alejandra Morales. All staff members have the required background clearances but two of the staff members are not associated to the facility. This is a repeat violation as the last citation was issued on 02/20/2019. Civil penalty of $200 will be assessed today.
“Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. “
A change of Site Supervisor packet shall be submitted to Licensing within 15 days.
Appeal Rights (1/16) were discussed and provided. Signature at the bottom of this report confirms receipt. Notice of Site Visit was posted during this visit and will remain posted for 30 days. |
