On 11/12/21 @ 10:45AM, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced inspection. The purpose of this inspection is to observe the corrections to the deficiencies cited on 10/21/21. Observed present today were 30 children with staff Felicia Blamo, Arianne Sanchez and Shelley McDole.

The following corrections were observed;

1. Health Related Services - Ms. Foster conducted a training with staff regarding regulation requirements in reporting injury to parents.

2. Buildings and Grounds - The following corrections were observed:
- Holes in the fenced have been patched.
- Holes in the play that poses tripping hazard have been covered.
- Barricades have been repaired and are now anchored safely.

No deficiency noted today.
