13 | On 07/14/2025 at 11:30AM, Licensing Program Analyst (LPA) Adriana Macias conducted an unannounced inspection for the purpose of delivering findings on a complaint received on 05/23/2025, with above allegation. Upon arrival, LPA was greeted by owner, Shelly McDole and toured the facility. A total of 31 children and 5 staff were observed with appropriate capacity and ratios. All staff have been fingerprinted and associated. During LPA’s initial inspection visit on 05/29/2025, children’s records were reviewed and found to be complete, however, there it was later determined that although a parent that was listed as an emergency contact on CCLD’s required Identification & Emergency Information Form LIC700, that same parent was removed from the Brightwheel application where the facility reports daily activities and any incidents that may occur during the day. As Licensing does not require that the facility use an application such as Brightwheel, it cannot be determined whether removal from the app would create any issues with contact. LPA conducted interviews and reviewed documentation. There were contradictory statements whether the app or the form is used for emergency contact.
Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, therefore the above allegation is found to be Unsubstantiated. Exit interview conducted and report was reviewed with Shelly McDole. A notice of site visit was given and must remain posted for 30 days. LPA observed form posted. |
