On 11/13/24 at 12:15PM, Licensing Program Analyst (LPA) Luigi Gargaro conducted an unannounced annual inspection. Upon arrival, LPA met with facility director Lisa Phipps. This is a full day program which operates year-round. Days and hours of operation are Monday – Friday 7:45am – 5:15pm. The director and LPA toured the infant room and playground. The following ratios were observed for the sole infant classroom: five children were being supervised by teacher Angelica Harrison and aide Elena Galura. All staff members have the required background clearances and are associated to the facility

Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and drinking water is available for the children. Storage areas are readily available, and the rooms accommodate class size. Napping equipment consists of cribs for younger infants and napping cots for the older infants. All changing tables are located next to respective sinks. Parents provide the children’s snacks and meals that are stored and prepared on-site. Cleaning supplies are kept out of reach of children.



Outdoor area for infants is separate from preschool outdoor area and has sufficient shade. The facility is free of flies, insects and rodents. Play equipment is age appropriate for the children in care. Drinking water is available both indoors and outdoors with the use of children's individual water bottles that are filled with water from filtered facility water dispensers.

LPA reviewed sign in sheets and medication policy and storage. Director confirmed that currently no infants in care have any medication that is being administered to them. Isolation area is the facility office. Personnel and client records were reviewed. The facility maintains an updated needs and services plan for infants in care after their initial assessment and an individual safe sleeping plan for the younger infants (12 months and under).

All staff members completed the required mandated online child abuse training. Facility has at least one staff on site with a valid EMSA approved pediatric CPR/First Aid certificates. LPA reviewed Emergency Disaster Plan and last fire and earthquake drills were conducted on 10/30/24. Facility has an operating carbon monoxide detector located in the infant center.

This facility currently is not dispensing any medication to children in care. Licensee is aware that an Incidental Medical Services - IMS - plan is required to be submitted to the licensing office if they provide any of these services. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Licensee representative was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. The ABC’S of Safe Sleep: Sleep is Safest: Alone, on their Back in an empty Crib on a firm mattress.

Child Care Providers can now sign up for Quarterly Updates and PINS through the DSS website at https://cdss.ca.gov/inforesources/community-care-licensing/subscribe. LPA discussed California Megan's Law with provider and advised her to go on the website at www.meganslaw.ca.gov.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

All unusual incident reports shall be submitted to Licensing office via email at SDIncidentReports@dss.ca.gov or via fax at (619)767-2203.

A notice of site visit was provided and to be posted at the facility for 30 days. Failure to keep notice posted will result in a civil penalty of $100.00.

The spray bottle was removed and no other cleaning supplies were found in any other cabinets in the classroom. Director understands that all cleaning products must always be made inaccessible by use of a latch or placed out of reach of children. Violation was cited as a Type B one as cabinet doors and cleaner were closed and doors could only be moved by gripping punch out holes with some effort.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.
