25 | Licensing Program Analyst (LPA) Keely Messerschmidt arrived at the facility on a case management inspection to follow-up on an Unusual Incident Report (UIR) which occurred on July 6, 2023 per Owner. LPA met with Director Taylee Spurlin, and provided purpose of inspection. At the time of inspection, LPA toured the facility, took census, interviewed and reviewed documents previously submitted to the department with Director.
The reported incident took place on July 6, 2023, regarding a child that had fallen off the slide injuring their clavicle.
LPA interviewed Director and confirmed that Child #1 (C1), was taken to the doctors on the following day from the incidence occurrence 7/7/23. Parents contacted facility informing them of the doctors visit and that C1 had a fractured clavicle. The following Monday 7/10/23 C1 returned to school and parents provided doctors report to Director with instructions on C1's physical activity restrictions. LPA also reviewed C1 file, observing the doctors printout. Director showed LPA message that was sent through their Brightwheel app to parent informing them of incident, no incident report on file. LPA confirmed that on 7/6/23 there were 12 children in attendance including C1 with 2 teachers, incident took place on the playground. LPA observed that the slide was removed from the playground. LPA confirmed that the owner had removed the slide that was bolted to the ground, as this was a second incident that had taken place and wanted to avoid any future issues.
An exit interview was conducted with Director Taylee Spurlin and a copy of this report was provided along with the Notice of Site visit.
