25 | On 10/20/21 at 3:25 PM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management visit at Licensee Christine D'Amico's request. The purpose of the visit was to approve and add classroom #2 to the school-age license. Upon arrival, LPA called Director Christine D’Amico. Ms. D'Amico stated that staff member Elisha Freese would meet with LPA. Elisha Freese (Medina) directed LPA to Open Minds classroom area and proceeded to pick up the children from classrooms as the school day had not yet ended. Ms. Freese stated they are only using room #1 currently and outside play area. Total census today was: 45 children. Staff members present were Elisha Freese, Marissa Angel, Yazmin Zapata and Niurka Osorio.
Fire clearance was granted on 10/12/21 to add Room #2 to current license approved for Rooms #1, 3, 10, 11 and Montezuma 1 to serve school-age children. There is no change to the capacity.
During today’s inspection of the facility, LPA observed furniture and equipment in good condition and free of sharp, loose or pointed parts. Disinfectants and cleaning solutions were inaccessible to children. Ms. Freese states there are no bodies of water or weapons on the premises. Ms. Freeze stated none of the children currently have medication but if so, it is stored in a locked file cabinet in the office. All floors appear clean and safe.
Children bring their own snacks and no food is stored at the facility. There are no signs of rodents or pests. All storage containers for solid waste have tight-fitting covers that are in good repair. Drinking water is readily available both indoors and out. Outdoor activity space has not changed. LPA observed playground equipment in good conditions and free of hazards. Bathrooms are clean and sanitary and allow for privacy. Facility has a working carbon monoxide detector. Staff and children’s records were not reviewed. Based upon inspection of classroom #2 and facility, LPA Lane has approved classroom #2 for use effective tomorrow 10/21/21. LPA Lane will mail updated license to CEO Christine D'Amico.
No deficiencies were cited.
An exit interview was conducted with staff member Elisha Freese. The Notice of Site visit was posted during today's visit and must remain posted for 30 days. |
