1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On January 21, 2021 at 3:13 PM Licensing Program Analyst (LPA) Adrian Mangina conducted an unannounced Case Management Inspection via Facetime in reference to a report that the facility is not informing parents of their children’s possible exposure to Covid-19. LPA met with Director Ana Lopez and proceeded to tour the facility. Present today were 7 children and 2 staff members. Appropriate ratios were observed. All staff members have the required background clearances and are associated to the facility. The Department received an anonymous report that a several others parents whose children attend the school parents of students have tested positive for Covid-19, one of whom came to the school to pick up the child and that the children of those individuals have not been excluded and the school has not closed. The Director stated that to her knowledge there were no parents in Caterpillars room that tested positive for covid-19 since March. Director did not have any covid-19 positive cases in those classrooms and did not contact Health department for facility #376701061. Director stated that the only positive Covid-19 exposure case was in facility #376701061. The Director states that the Department of Public Health, staff and all parents with children enrolled at this the associated facility (license #376701061) were notified of the Covid-19 exposure via Brightwheel messaging system on 1/11/21. Room/Age/number of children/staff members Room 1 – Caterpillars there were 7 children present with 2 teachers Angie Hernandez and Itzelt Garcia Facility appears to be following COVID-19 guidelines and no immediate hazards were observed by LPA. LPA Mangina advised Director to report covid-19 positives or exposures within 24 hours to the duty and follow up with a Unusual incident report within 24 hours and include a copy of the facility roster. Covid-19 guidelines visit was conducted by LPA Tyra Block on 6/5/2020.

COVID-19 guidance was reviewed with Director Lopez and she was provided resources via e-mail: COVID-19 Guidance, CAL-OSHA guidance, Childcare Decision Tree, Covid-19 Self-Assessment, posters to display, list of websites for more information and a link to a mask video for children.

No deficiencies were observed.

An exit interview was conducted with Licensee. A Notice of Site Visit (LIC9213) and Appeal Rights (LIC9058) will be sent along with the report (LIC809) via e-mail to the Licensee. Licensee will confirm receipt of these reports via e-mail and the reply of confirmation will serve as the signature acknowledging these rights. The Notice of Site Visit (LIC9213) must remain posted for 30 days.
