13 | On 08/10/21 at 12:51pm, Licensing Program Analyst (LPA) Casey Gulley conducted an unannounced inspection to deliver complaint findings for the above allegations. LPA Gulley met with Concepcion Flores and discussed the purpose of the inspection. It was alleged that Staff did not follow proper reporting requirements for a daycare child while in care.
During the course of the investigation, LPA Gulley conducted an unannounced tele-inspection. Interviews were conducted with multiple daycare parents, facility staff, social worker and licensee. Staff denied allegation and stated children are screened daily and any concerns are documented and discussed with parent or guardian. Staff documented minor bruising to Child #1’s (C1) wrist and nose on 4/1/21 and 4/29/21, but didn’t feel the injuries were out of the ordinary for the child’s age. A review of medical records and photos indicate C1’s injuries were non-specific, in that, they could have been caused by either accident or inflicted trauma.
