25 | On October 27, 2021 at 8:55 a.m. Licensing Program Analyst, Leilani Curtis, conducted an unannounced inspection to follow up on a self-reported incident that occurred on 9/20/21. Upon arrival LPA met with Director Katrina Wanemacher and proceeded to tour the facility. There were 52 children present with 7 staff members. Appropriate ratios were observed. Staff members have the required background clearances and are associated to the facility.
On Monday, 9/20/21 at approximately 10:40 a.m. the Preschool Class was transitioning from the playground to their classroom when a child (C1) left the line and hid out of sight under a play structure. The class continued to the classroom without C1. At approximately 10:50 a.m. staff #1 (S1) saw C1 under the play structure alone, without supervision. S1 called the child over to join her class (Two’s Class). After C1 joined the Two’s Class the child’s teacher (S2) and the director were notified.
During today’s inspection interviews were conducted with several staff members. At the time of the incident there were 19 children and 2 staff on the playground. Appropriate ratio was in place. Information gathered indicates that C1 was left alone on the playground for approximately 10 minutes. The child returned to care after the incident. The director states that the mother of C1 and the District Leader were notified of the incident. The director also states that retraining on transitions & supervision occurred on 9/27/21 for the classroom staff involved and on 10/11/21 for all facility staff. A plan of action was put into place to prevent this from happening again. In the future if a classroom is having a difficult transition the classroom will break into two groups and transition separately or stop the transition and regain control before proceeding.

An exit interview was conducted with the director and appeal rights (LIC 9058 1/16) were discussed. The director’s signature on this form acknowledges receipt of these rights. LIC 9213 NOTICE OF SITE VISIT FORM IS REQUIRED TO BE POSTED FOR 30 DAYS. LPA observed the director post notice of site visit. |
