On 9/26/22 at 10:05 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management inspection in regards to a self-reported incident that occurred on 8/12/22. Director Kristina Simon reported that 10 preschool children and one staff member were trapped inside the Limited Access Limited Use (LULA) elevator for approximately 5-10 minutes. Director called 911 and SDFD arrived within 5 minutes and opened the elevator. No children were injured and parents were notified regarding the incident and SDFD on site. The LULA was put out of order until further notice while awaiting repair and signs were posted.

Upon arrival, LPA Lane met with Center Director Kristina Simon and toured the facility. Census was 48 children in 3 classrooms with 6 staff members. Appropriate capacity and ratios were observed. Director stated that the LULA is now operational but the phone inside is not functioning according to the elevator company standards. Elevator company (McKinley) came out last week to repair the elevator, however they are still working on making the phone ring louder. The company stated that the elevator can be used but for staff to carry a handheld phone while in the elevator. Director stated she would send the repair order to LPA via e-mail. Director stated that they use the LULA to transport the 2-year old children to the playground on the upper level as a safety precaution as there are 2 full flights of stairs. Only the playground is on the upper level. The 3-year old and 4-year old classrooms use the stairs to get to the playground. LPA observed the inside of the elevator and took pictures of the documentation inside.

LPA advised Director to send final documentation once phone is operating according to the elevator company standards. No deficiencies were cited today.
Exit interview conducted and report was reviewed with the facility representative Director Kristina Simon. A notice of site visit was given and must remain posted for 30 days.
