On 6/21/21 at 1:00 PM, Licensing Program Analyst (LPA) Keturah Lane and Licensing Program Manager (LPM) Monica Cuddy visited the facility to conduct an annual inspection. Upon arrival LPA/LPM met with Director Christine D’Amico and proceeded to tour the facility. During today's inspection, there were 7 children with 2 staff members in one classroom. Appropriate ratios and capacity were observed. Appropriate care & visual supervision were also observed during the inspection while children were playing outside in the turf area next to the classroom.

Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation. Floors appear to be clean and safe. Drinking water is readily accessible. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. There is a sink area for washing hands and a small refrigerator used for snacks as needed. Chilldren bring their own lunches and snacks from home. Cleaning supplies are kept separate from food and are inaccessible to children. Storage containers for solid waste have tight-fitting covers and are kept in good repair. There are no hazardous items or poisons accessible to children. Director stated there are no firearms or other weapons on the premises. The facility appears to be free of insects and rodents. There is an operational carbon monoxide detector at the facility.

There are two outdoor play areas. The main playground contains equipment that is appropriate for ages 5-12 years upon inspection. LPA Lane advised that only the 5 years-olds use the play structure and staff supervise younger children in that area so they do not use the structure. The second outdoor play area is located close to the classrooms and contains a small turf area and picnic area. This area was remeasured and total area is 895.6 square feet which comfortably accommodates 11 children. Total outdoor play space available is 3,607.12 square feet which is large enough for 48 children. (continued on LIC809-C...)

Outdoor play area is fenced with sufficient material for cushioning. There are no bodies of water at this facility. Climbing structures, swings and slides are securely fixed to the ground. Area has umbrellas and trees used for shade. Area has drinking water readily accessible and grounds are free of debris or potential hazards.

LPA reviewed medication storage. Personnel records contain documentation of education and at least one staff member has current CPR and First Aid certifications. Two staff members have not renewed mandated reporter training or had proof of immunity to the flu or a statement opting out in their file. Sign-ins were reviewed via online system called Sandbox. Children’s records contain emergency contact information. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions.

Pursuant to Title 22 of the CA Code of Regulations, the following Type B deficiency was cited (refer to LIC 809-D).

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies and no records were reviewed as child with medication was not present at the facility. LPA Lane discussed information regarding the stipend with Director and obtained up to date contact information.

An exit interview was conducted with the Director. The Director was provided a copy of their appeal rights (LIC 9058) along with the report (LIC809) and their signature on this form acknowledges receipt of these rights. The LIC 9213 (Notice of Site visit) was posted during today's visit. Notice of Site Visit must remain posted for 30 days.
