25 | On 12/02/22 at 11:01am, Licensing Program Analyst (LPA) Samantha Clenista conducted an unannounced case management site inspection to follow up on a self-reported incident that occurred on 11/09/2022. LPA's initial visit to address this incident was on 11/30/22.
Upon arrival, LPA met with Director, Christine D'Amico, and proceeded to tour the facility. There were a total of 15 children with a total of 4 staff members. Children in care were observed playing indoors. Appropriate ratio, capacity and supervision was observed during inspection.
On 11/09/2022, facility reported that Child #1 sustained a broken clavicle from another child who stood up and hit his head against Child #1's collar bone during outdoor play. On LPA's initial visit conducted on 11/30/22, LPA observed where the incident occurred. Area was observed was safe, age appropriate and free of hazards. Ms. D'Amico provided LPA with related documentation prior to thart inspection. LPA interviewed staff and children during that inspection. During today's inspection, LPA conducted an interview with an additional day care child and day care parent.
Based on information gathered, LPA confirmed that no violations have been identified and that the injury Child #1 sustained was an accident. See 9102 for Technical Assistance.
LPA reviewed this report with Ms. D'Amico at conclusion of visit. LPA provided a copy of LIC 9213, “Notice of Site Visit,” and observed Ms. D'Amico post the notice during visit. Notice must remain posted for 30 days. |
