25 | On September 4, 2024 at 2:00pm, Licensing Program analyst (LPA), Daniela Huerta conducted an unannounced case management inspection to follow-up on a self-reported unusual incident reported to the Department. Upon arrival, LPA met with Assistant Director, Shannon Hontz and disclosed the purpose of the inspection and was led on a tour of the facility. There were 33 children present, and 5 staff members present. Hours of operation are Monday through Friday 6:00am - 6:00pm.
The Department received an unusual incident report on August 6, 2024, in which an unusual incident occurred on August 5, 2024 when former staff member (S1) engaged in a physical altercation with two (2) other staff members (S2 & S3) in the facility lobby.
LPA inspected the lobby area as well as the classrooms where the children were present during the incident and found the area to be visually and physically separate from the classrooms. LPA interviewed staff and daycare parents that were present during the time of the incident. Facility staff prevented LPA from interviewing children regarding the incident, which was cited during an inspection conducted on August 13, 2024. LPA reviewed surveillance video of the incident. Based on the interviews conducted and review of the video footage, it was determined facility staff reacted to the unexpected incident appropriately and quickly, guiding the children inside the lobby away from the altercation and into the classroom area. Based on information obtained no citation for personal rights violation is warranted.
LPA reviewed the facility Admission Agreement and Parent Handbook with assistant director and reiterated the department’s inspection authority. LPA consulted with the assistant director regarding the facility’s current termination practices in efforts to ensure future similar incidents do not reoccur. |
