13 | On 05/12/2022 at 10:50 am, Licensing Program Analysts (LPAs) Michelle Hood and Cindy Meier conducted an unannounced complaint inspection and met with the Director. LPAs disclosed the purpose of the inspection and were granted entry into the facility by the staff LaKisha Davis. LPA Meier toured the facility. Eight (8) pre-school children with one (1) staff were present in the facility during this inspection.
During the investigation, interviews were conducted with the licensee, director, staff, daycare children, daycare parents and witnesses. Based on the interviews conducted with the licensee, director, staff, daycare children, daycare parents and witnesses stated daycare children are supervised by staff. The facility serves pre-school children ages 2.5 - 4.5 years in one preschool classroom. The facility may serve up to 12 school-age children. The combined capacity shall not exceed a total of 28 children.
Due to conflicting statements obtained during the investigation, the above allegation was found to be UNSUBSTANTIATED meaning that although the allegation may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred.
The director was provided appeal rights (LIC9058 01/16) and their signature on this form acknowledges receipt of these rights. Notice of site visit was provided to the director and must remain posted for 30 days. An exit interview was conducted, and the report was reviewed with the director Ivette Martinez.
No deficiencies cited.
