1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 12/1/20 at 8:15 AM, Licensing Program Analysts (LPAs) Keturah Lane and Patrick Ma conducted an unannounced case management tele-inspection due to COVID-19 State of Emergency via Zoom with Director Marla Martin. The purpose of the visit was to inquire about recent positive cases of COVID-19 at the facility and compliance with COVID-19 guidelines. There is also an infant license at this facility #376701401 that has not been impacted by the Covid-19 positive cases. Director stated that following census for classrooms under the preschool license: Bumblebees (Toddlers) - 0 children, classroom is currently closed due to COVID-19 Grasshoppers (After school program) – 0 children, classroom is currently closed due to COVID-19 Caterpillars (3 yr-olds) – 9 children with staff member Michelle Antunez Dragonflies (Pre-K) – 5 children with staff member Alejandra Mejia Fireflies (Pre-K) 8 children with staff member Vanessa Godinez Ladybugs (2 yr-olds) – 9 children with staff member Carla Delhotal On 9/21/20, the facility reported 2 positive cases in the Dragonfly classroom which closed on 9/21/20 and reopened on 10/1/20. There was also 1 positive case in the Caterpillar classroom and that classroom was reopened on 10/12/20. On 11/24/20, Licensing was notified by facility of one COVID positive staff member who worked a few hours prior to receiving positive results. Per Director all affected parents have been notified of all positive test results and Director closed the classroom. Director tried to call Public Health Dept on Monday 11/23/20 and received call back on 11/24/20. Per public health the facility was advised to close the affected classroom (Bumblebees) which had already been done and to notify all affected families. Bumblebee classroom will reopen on 12/8/20. Director had an additional positive covid report from another staff member on 11/29/20 and was in the process of reporting it to Licensing. Staff member had last been at facility on Wednesday 11/25/20. Director contacted health dept on 11/30/20 and was advised to close the classroom and notify parents. Classroom closed is Grasshoppers (school age) and will reopen on 12/10/20. Facility is to submit a written unusual incident report (LIC624), facility roster (LIC9040) and updated personnel report (LIC500) by Thursday 12/3/20.

Director stated that an additional staff member also tested positive a couple of weeks ago, but she hadn’t been at the facility during the time of exposure. LPA reminded Director to report all positive covid-19 test results of staff, children and parents/guardians regardless of when they were last in contact with the facility to the health department and Licensing. Director stated she contacted the health department on both 11/24/20 and 11/30/20 regarding the latest two staff members that tested positive.

Staff shortages required the Director to end the meeting at 9:07 AM to attend to children in the open classrooms. Meeting will be continued via FaceTime at 2:30 PM today, 12/1/2020.
