Licensing Program Analyst (LPA) Diana Sanchez, made an unannounced complaint inspection today to deliver complaint finding on the above allegation. LPA met with Anahit Antonyan and made her aware of the reason for today’s inspection. Current census is 25.

This agency has investigated the complaint alleging facility staff placed children to nap on a Boppy type pillow. During the investigation, LPA reviewed video footage, interviewed facility staff and parents. LPA inspected the facility on 2/07/2020 at 2:35 p.m. and observed five infants laying on top of Boppy type pillows in infant classroom #6. Three of the infants were asleep. LPA reviewed video footage for the three napping infants and one of the infants was observed asleep face down on the Boppy type pillow at beginning at 1:40 p.m. Staff admitted to placing children to nap on the Boppy pillows, but stated they transfer the children to a crib once they fall asleep. A review of the manufacturer’s recommendations indicates children are to never be allowed to sleep on Boppy pillows.

Based on LPA’s observations and staff interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & Chapter 1), are being cited on the attached LIC 9099D.”)

An exit interview was conducted with Anahit Antonyan and a copy of this report and LIC-9099D left at the facility as well as appeal rights. During this evaluation, LPA advised Anahit that all request for extensions of any citations/Proof of Corrections (POCs) must be made within 10 days to the issuing LPA on or before the date the POC is due. Appeals to citations must be made within 15 days in writing to the issuing LPA's supervisor on or before the date the POC is due.

Facility director stated that they had a meeting with staff and provide them additional safe sleep training. Director provided a copy of the staff training outline with each staff signature as proof of correction. No additional plan of correction needed. Deficiency has been cleared.

Based on LPA observations and staff interviews the licensee did not provide a crib for each napping infant which poses an immediate Health and Safety risk to the children in care.
