25 | On 6/4/21 at 1:15 PM Licensing Program Analyst (LPA), Adrian Mangina made an unannounced visit to follow up on a self reported incident that occurred on 5/26/21 wherein a child (Child #1) removed the back lid off the toilet, dropped it resulting in a minor injury to the child..
LPA toured facility and obtained relevant documents. Census was 16 at time of visit. LPA observed six children and three staff in the infant room and ten children and three staff in the toddler room. Interviews and observations were conducted. Based on interviews and observations it appears that Staff #1 was within arm’s reach putting soiled clothing in a bag when Child #1 lifted a toilet tank lid once, was directed by the teacher not to do that again but lifted it a second time, higher than the first (approximately one to two inches) and when the lid dropped a small corner piece broke off cleanly causing a minor injury to the child. Parents were notified by telephone and child was picked up shortly after and received medical attention.
There was one child (child#1) and one teacher (Staff #1) present in the bathroom at the time of the incident. Director stated that she blocked access to the toilet until the tank lid was replaced. Director now instructs staff to have children not actively using the toilet sit in the “waiting chairs” once they finish going potty, while teachers are bagging dirty clothes. In addition, Director is investigating other methods such as straps to secure the toilet lids to the tanks to prevent similar incidents in the future. The bathroom toilets appeared to be in good repair.
Supervision was in place, ratios were met, the facility responded appropriately and reported timely. No deficiencies are cited.
