25 | On 1/9/24 at 12:35 pm, Licensing Program Analyst, Gerald Poindexter, made an unannounced visit to follow up on a self-reported incident that occurred on 12/14/23 wherein Child #1 (C1) tripped, fell, and hit their head on the green-painted flowerbed in the infant Playground Area 3, sustaining an injury. LPA met with Director Vivian Bennett.
LPA examined the area of the fall, inspected the brick flowerbed, and interviewed the staff closest to the incident and who attended to the child. LPA also interviewed Ms. Bennett. There were 5 staff members with 14 infants at the time of the incident. Based upon information obtained today, Staff #1 (S1) directly observed the fall but was unable to intervene quickly enough to prevent it. She verified the staff/child ratios and that she administered first aid to the child.
Play Area 3 has turf surface in good condition, age-appropriate equipment, and direct supervision was in place. The facility has a waiver to share the playground at different times with the Toddler component on a schedule basis. Ratios were met and staff responded appropriately with first aid, limited activity for the child, and called the child’s mother. The extent of the injury was not immediately apparent, and child’s parents sought medical attention later that evening as a precaution. The child received treatment and returned to the facility the following day without any ongoing complications.
Since the incident, Ms. Bennett states (and LPA observed) that the flowerbed area in Play Area 3 was reinforced with rubber padding along approximately three-quarters of its surface. The remaining area is used to place water bottles and other items, as barricade, of sorts.
No deficiencies are cited. Exit interview conducted with the director Vivian Bennett.
