Licensing Program Analyst (LPA) Yolanda Baez arrived at the facility to conduct a pre-licensing inspection. LPA met with Applicant/Director, Desiree Fajota. This Infant program operates in the "Infant" classroom and the Toddler component operates in the "Toddler" classroom (located inside of the same class as the Infant but are kept physically separate through the use of a barrier) and both the "Infant" and "Toddler" classrooms are located within Classroom 113. The hours of operation are Monday - Friday from 7:00am to 6:00pm. This center is located within the same premises as the Good Shepherd Parish.

The "Infant" and "Toddler" classrooms were inspected and measured during this inspection and measure the following:
  • Infant: Infant Class (ages 0 -18 months): measured a total of 265.5 square feet
  • Toddler: Toddler class (ages 18 to 36 months): measured a total of 426.08 square feet


The Infant class has a separate sleeping area (cribs) and it is separate from the activity area. The sleeping area is not included in the indoor square footage calculation. The "Infant" classroom section is equipped with age appropriate equipment, furniture, supplies, materials, and refrigerator. There is one sink in the Infant section and it will only be used to sanitize hands after diaper changing. Applicant understands that the sink that is used to sanitize hands after diaper changing cannot be used to clean and disinfect the Infant eating equipment and bottles. Per Applicant, parent's will be in charge of taking bottles and eating equipment home to clean and sanitize. There is one changing pad in the Infant class and it is within arm's reach of a sink. There is a separate restroom that is equipped with one toilet and one sink that is located near the Nurse's office and it will be used for an isolation bathroom for children who are ill. Because the Infant classroom is able to accommodate 7 infants instead of the original requested capacity of 8 infants, Applicant made changes to the LIC 200A to update the new requested total for the Infant children and agrees to license for 7 infants and 12 toddlers. The Director's office will be used as an isolation area for children who are ill. There is a carbon monoxide detector, smoke alarm, and first aid kit with required supplies.

Disinfectants, cleaning solutions, and other hazardous substances are kept in locked cabinets. Per Applicant, parent's are responsible for providing lunches and snacks. The center will be providing hand written sign in and sign out. All required documents are posted.

The "Toddler" classroom section does not have their own toilet, per Applicant children who will begin potty training will be transitioned to the Preschool component. There is a changing pad located in the Toddler class and it is within arm's reach of a sink. The "Toddlers" classroom is equipped with cots/mats, age appropriate equipment, and materials for children. The entrance to the Toddler component is through the Infant classroom and divided by a partition. The ratio in the "Toddler" classroom will be 1:6.

The facility currently has two separate outdoor activity spaces, one is for the Preschool program and one is for the Infant program. The Infant play yard is located directly next to the Preschool play yard and measured a total of 1,209.73 square feet which is sufficient for 16 children at one time. The Infant play ground has trees used for shade. Children have access to water outdoors via children's water bottles that are brought from home. Applicant is to install a sturdy partition to separate the preschool program from the Infant program and it must be free of sharp edges and hazards. LPA and Applicant discussed proper supervision and placement of staff on the play yard to ensure the safety of children at all times. Applicant was advised that the infants and toddlers cannot commingle at any time during indoor and/or outdoor activities.

This facility plans to provide Incidental Medical Services – IMS and has submitted a Plan of Operation. The following requirements are pending and must be received prior to issuing a license:
  • Install a barrier to physically separate the Preschool and Infant play yard
  • A play ground waiver to allow the infants and toddlers to use the Infant playground at separate times must be submitted and approved prior to issuing a license
