Licensing Program Analyst (LPA) Yolanda Baez arrived at the facility to conduct a pre-licensing inspection. LPA met with Applicant Christine D' Amico. This School Age program operates in classrooms 305, 304, and the Library. The hours of operation are Monday - Friday from 7:00am to 8:45am, from 3:35pm to 6pm on Mondays Tuesdays Thursdays and Fridays, and from 12:50pm to 6pm on Wednesdays. The program is located within Adams Elementary School.

Classrooms 305, 304, and the Library were inspected and are equipped with appropriate furniture, tables, chairs, bookshelves, and other materials for the children. The children have access to two restrooms that are gender specific (located to the right of Room 304, if facing room 304). There is a Girl's restroom that consists of 4 toilet stalls and 7 sinks. There is a Boy's restroom that consists of one toilet stall, 4 urinals, and 7 sinks. The children also have access to a restroom located in Room 305 that is equipped with one toilet and one sink. There are more than sufficient toilets and sinks for the requested capacity of 30 school age children. The children will be escorted and supervised to and from the bathrooms. Drinking water is available in all the classrooms via drinking fountain and water pitchers with cups. Disinfectants, cleaning solutions, and other hazardous substances are kept inaccessible. The center will be providing snacks and the menu is posted. All required documents are posted. The center will be providing electronic sign in and sign out. There is an operational carbon monoxide detector and a smoke alarm. The first aid kit is kept in Room 305 and tweezers and scissors must be added to the first aid kit. The "Quiet area" in Room 305 (near the restroom that is located in Room 305) will be used for the isolation area for an ill child. The restroom that is located in Room 305 consists of one toilet and one sink and will be used for any child that is ill. Applicant stated that in the event that they would use the restroom in classroom 305 for an ill child, they will have the children use the restrooms that are located near room 304 and will properly sanitize the restroom located inside of room 305.

The playground area is located in front of classroom 304 and includes several age appropriate play structures, a swing set, trees for shade, a proper fence, and sufficient cushioning material. Drinking water is readily available outdoors via water fountain. There is more than sufficient outdoor activity space to accommodate the requested capacity of 30 children. Proper supervision and placement of staff on the play yard was discussed to ensure the safety of children at all times. The facility may be sharing the outdoor play area with Prime time after school program and has submitted a Plan of Operation for both the outdoor play space and the restrooms to ensure that children from OpenMinds center will never co- mingle with the children from other after school programs. Applicant made an update to the facility sketch to take away the "Lunch Arbor" as an accessible area due to it being accessed by the public. The areas that will be utilized by the center are classrooms 305 and 304 and the Library. Because the Library is currently being used to temporarily house administrative staff from Adams Elementary School, Applicant understands that children cannot have access to the Library until the Adams Elementary School staff are no longer being housed in the Library.

This facility plans to provide Incidental Medical Services – IMS and has submitted a Plan of Operation. A fire clearance for 30 school age children was granted on 08/01/19.

The following corrections must be made prior to issuing a license:
  • Purchase and send proof of purchase to LPA of missing requirements for the first aid kit which include tweezers and scissors
  • LPA must request an updated STD850 from the fire department due to Applicant adding Rooms 305 and 304 for the children's use and taking away the "Lunch Arbor"
