Licensing Program Analyst (LPA) Yolanda Baez arrived at the facility to conduct a pre-licensing inspection. LPA met with Applicant Timothy Captain and Director Kim Holloway. This infant program operates in the "Infant 1" and "Infant 2" classrooms. The toddler component operates in the "Toddlers", "Two's 1", and "Two's 2" classrooms which is located within the same building as the Infant Program. The hours of operation are Monday - Friday from 6:00am to 6:00pm.

The "Infant 1", "Infant 2", "Toddlers", "Two's 1", and "Two's 2" were inspected and measured during this inspection and measure the following:
  • Infant 1: Infant Class (ages 6 weeks to 24 months): measured an estimated 269.23 square feet. The Infant 1 room is missing a partition to physically separate the napping area and the activity space area and will have to be measured again to gain a more accurate measurement once the partition is installed.
  • Infant 2: Infant Class (ages 6 weeks to 24 months): measured an 391.64 square feet.
  • Toddlers: Toddler Class (ages 18- 36 months): measured 376.04 square feet
  • Two's 1: Toddler Class (ages 18- 36 months): measured 381.36 square feet
  • Two's 2: Toddler Class (ages 18- 36 months): measured 373.49 square feet

There is to be a separate sleeping area (cribs) in the Infant 1 classroom and a partition or barrier at least 4 feet in height will have to be installed in order to make the sleeping area physically separate from the activity area. The sleeping area will not included in the indoor square footage calculation. The Infant 1 room is equipped with age appropriate equipment, furniture, cribs, cubbies, supplies, and materials. There is one changing table and one sink in the Infant 1 class, Applicant understands that the one sink cannot be used for sanitizing hands and washing bottles/other infant eating equipment. Infant 2 classroom is equipped with age appropriate equipment, furniture, cots, cubbies, supplies, and materials and consists of one changing table, one toilet, and one sink. ...CONTINUED ON PAGE 2...

The "Toddlers, Two's 1, and Two's 2" classrooms all are equipped with age appropriate equipment, furniture, cots, cubbies, supplies, and materials. The "Toddlers" class has one toilet and 2 sinks available for children. The "Two's 1" and "Two's 2" classrooms are each equipped with one changing table (within arms reach of a sink), one toilet, and two sinks. The ratio in the "Toddlers, Two's 1, and Two's 2" classrooms will be 1:6.

There is a carbon monoxide detector, smoke detector, and first aid kit with required supplies. Disinfectants, cleaning solutions, and other hazardous substances are kept inaccessible. The director's office will be used for the isolation area for an ill child. There are two staff restrooms located in the hallway, near the reception area. The staff restroom that is directly across from the reception area will be used for children who are ill. All required documents are posted on the parents' bulletin board. Center has menus posted and drinking water is readily available inside of the classrooms via water bottles brought from home.

The facility has outdoor activity space for the infant program and the toddler component however, there are not two physically separate play yards only one big play area. The infant's/toddler's play yard is located directly outside of the "Two's 1 and "Toddlers" class (in the middle area). The Infant/Toddler play ground measured 834.80 square feet, which is sufficient for only 11 infants/toddlers at one time. The infant/toddler play yard is complete with age-appropriate play equipment, shade, sufficient cushioning material, and is surrounded by a 4 feet high fence. The infants and toddlers will be scheduled on the playground at different times. This facility plans to provide Incidental Medical Services – IMS and has submitted a Plan of Operation. Applicant is requesting a waiver to allow the infants to share the play yard with the toddler aged children on a scheduled basis and no more than 11 children at a time. Proper supervision and placement of staff on the play yard was discussed to ensure the safety of children at all times. Applicant was advised that the infants and toddler children cannot commingle at any time during indoor and/or outdoor activities.

  • Fire clearance
  • 4 Foot high partition to separate the napping area in Infant 1 classroom
