1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 08/31/20 at 4:00 p.m., Licensing Program Analysts (LPAs), Michelle Hood and Rajani Goudreau and Licensing Program Manager (LPM), Tulam Vu arrived at the facility to conduct a pre-licensing inspection. LPA Hood met with Director of Quality Assurance, Barbara Saxton, Program Director, Patricia Smith and Director Wendy Santacruz. This preschool program operates in the Ladybugs and Butterflies classrooms. The Preschool program is located within the Finney Elementary School premises. Facility is Title 5, per verification from the Department of Education. The hours of operation are Monday - Friday from 6:30am to 6:00pm. Ladybugs and Butterflies Classroom were inspected and measured during this inspection and they measure the following: · Ladybugs Classroom: Preschool classroom (2 - 5 years): measured a total of 851.74 square feet · Butterflies Classroom: Preschool classroom (2 -5 years): measured a total of 1013.35 square feet The total indoor capacity is 1883.08 square feet which is sufficient to accommodate 53 children at one time. The classrooms are equipped with appropriate furniture and napping equipment (cots) including tables, chairs, cubbies, bookshelves, and other materials for the children. There are a total of three toilets and three sinks available in the Butterflies classroom and two toilets, one single sink and a sink with two faucets (outside of the restroom) in the Ladybugs classroom for the children's use which is sufficient to accommodate requested capacity of 48 children. At this time facility is only requesting 48 children. Children are to be escorted and supervised by staff to and from the bathroom and direct visual observation is required. Drinking water is available (in and outdoor) in the classrooms via water dispensers and disposable cups. There will be a space in the Directors office that will be used for an isolation area. There is a separate bathroom for staff located in the “Butterflies" classroom, it is equipped with one toilet and one sink, and is designated for use by children who are ill.

There is a functioning carbon monoxide detector in each classroom and a first aid kit with required supplies. LPA informed Barbara, Patricia and Director that disinfectants, cleaning solutions, and other hazardous substances shall be kept inaccessible. The kitchen will be used as a food preparation area and it currently includes one sink, one refrigerator, a stove, food supply, and storage units. There is a freezer in the storage room located in the “Butterflies” classroom next to the Directors office. The menu is posted. The center will be providing manual sign in and sign out. Required documents are posted on the parents' bulletin board.

The Preschool play yard and garden is separate from the classroom. The play area was inspected and measures 16,785 square feet which is sufficient for 223 children. The playground is shared with licensed Finney State Preschool. The play area is fully fenced and includes outdoor play equipment, age appropriate play structure is designed for ages 2-5, sufficient material (wood chips) for cushioning, a shaded area (3 Canopies), and drinking water. Drinking water is available in pitchers and cups. Proper supervision and placement of staff on the playground was discussed and advised with Barbara, Patricia and Director to ensure the safety of children.

This facility plans to provide Incidental Medical Services – IMS has submitted and approved. Once outdoor playground waiver is approved, a license for 48 children ages 2-5 years in classrooms Ladybug and Butterflies, may be issued after a file of review.
