On 11/08/21 at 9:20 a.m., Licensing Program Analyst (LPA), Rajani Goudreau arrived at the facility to conduct a pre-licensing inspection. Upon arrival LPAs, met with the President, Jennifer House, and VP of Operations, Jennifer Grawvunder, and proceeded to tour the facility. Applicant is requesting a capacity of 24 infant children; ages 6 weeks through 2 years old in the Elephant’s classroom. The facility’s planned days/hours of operation are Monday through Friday from 6:00 a.m. to 6:00 p.m.

The Elephant classroom was inspected and measured the following during this inspection:
· 6 weeks through 2 years old: measured a total of 1,047.72 square feet

Total indoor space for the infants is sufficient to accommodate 29 infant children. However, the facility is requesting a capacity of 24 infants. The infant classroom is located on the lower south side of the facility. In addition, the infant classroom has a separate parent entrance/exit located near the front of the classroom. The classroom is equipped with age appropriate furniture, including cribs for napping, tables, chairs, cubbies, bookshelves and play materials and for the children. There are stairs in the classroom leading to the first story of the facility. The stairs are gated and made inaccessible to children in care. Also, there is a laundry/staff room in the classroom that is securely inaccessible to the children by the use of a secure gate. The classroom has adequate heating, lighting and ventilation. The classroom is clean and orderly and are free of hazards. The classroom has a separate area for napping, including cribs and a rocking chair. Crib mattresses are maintained and in safe condition. The napping area was not included in the indoor square footage calculation. There are two sinks within arm’s reach of the changing tables in the classroom. The infant changing tables are padded with washable vinyl at least one inch thick with sides raised at least 3 inches. The director understands the sink near the diaper changing tables shall not be used for food preparation. There is another sink in the classroom available for the children's use. LPA informed the director there shall be one potty chair to every 5 potty training infants. There are two potty chairs available for the children’s use. In total there are three sinks and two potty chairs available for the children’s use, which is sufficient based on the requested capacity. All hazardous items; including disinfectants, cleaning solutions are stored where they are inaccessible to children. Director stated drinking water will be available to the children in care inside and outside via refillable water containers and cups provided by the facility.

There are two separate restrooms for staff located in the classroom. The staff restroom located on the first story of the facility and the director’s office will be utilized as an isolation area, if a child becomes ill. The isolation area shall be equipped with a crib, cot or playpen for each ill infant. In addition, infants in the isolation shall be under constant visual observation. There is a carbon monoxide detector in the classroom and a first aid kit with required supplies. Required documents are posted in the classroom near the parent entrance/exit. There is a kitchen located on the first story of the facility that will be used as a food preparation area and includes three sinks, one refrigerator, one freezer, a stove, food supply, and storage units. The outdoor activity space for infants was inspected and measured a total of 2,455.30 square feet which is sufficient to accommodate 32 infant children. There are two separate outdoor play areas for the infants; play area #1 and play area #2. The infant program has exclusive use of the outdoor activity space. The outdoor activity space is enclosed by a fence. There is a shaded rest area for the children by the use of removable canopy. The outdoor activity space is free of hazards. The outdoor activity space is equipped with age appropriate play equipment and toys. There are no climbing structures present in the infant outdoor activity spaces. Proper supervision and placement of staff outside was discussed to ensure the safety of children at all times.

This facility provides Incidental Medical Services - IMS. IMS plan submitted and approved. LPA discussed the following resources: Lead Positioning Brochure, Shaken Baby Syndrome and PIN 20-24-CCLD regarding Safe Sleep Regulations in Effect.

A fire clearance was granted on November 02,2021 for 24 infants; ages 6 weeks through 2 years old in the Elephant classroom.

An exit interview was conducted with applicant representatives. The following reports were discussed and provided: LIC809, LIC809-C and Appeal Rights (LIC9058). A license will be issued for 24 infant children; serving ages 6 weeks through 2 years old, after final file review. After final file review the license will be mailed to the facility and upon receipt shall be posed in a prominent place in the facility.
