At 2:40PM, LPA reviewed facility and children’s records. Children’s files were reviewed and included the: Identification of Emergency Information (LIC700); Notice of Parent's Rights (LIC995); Health History (LIC702); and Consent for Medical Treatment (LIC627).
At 3:00PM., Based on record review and interview, LPA confirmed licensee is missing immunization record and file for infant-age child, C1, in care.
LPA reminded licensee to document infant napping conditions every 15 minutes. Advisory Note: Technical Violation (LIC9102TV) was issued.
At 3:05PM., Based on record review, LPA confirmed licensee missing 'Individual Infant Sleeping Plan' for infant in care. Advisory Note: Technical Assistance (LIC9102TA) was issued.
Licensee’s 'Mandated Reporter Training' Certification (AB1207) was current: expiring 3/17/2024.
Licensee’s Cardiopulmonary Resuscitation (CPR)/ First Aid certification was current, expiring: 6/2024.
Licensee is conduct, and document emergency disaster drills once every 6 months, with last one done on: 1/31/2023. LPA reminded licensee to log every emergency disaster drills conducted.
Required posting are entry way: including Childcare License, Notification of Parent’s Rights (PUB379), and Emergency Disaster Plan (LIC610A). LPA reminded licensee to have posting visible for families.
Per licensee, isolation of ill children is in playroom.
Per licensee, facility provides snack, foods services for children in care. LPA reminded licensee to ensure all children’s meal containers brought by families should be labeled. Per licensee, home does not have firearms.
Licensee was reminded that all adults 18 years and over, living or working in the home, including employee and volunteers, must obtain criminal clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/ day up to $500.00 maximum per day/ per person will be assessed if this regulation is violated.
